Private Event Request

If you are seeking to host a PRIVATE OR EXTERNAL EVENT at Gethsemane United Methodist Church, please use this form. This should be filled out to request a Wedding, Funeral, Baptism, Meeting or other Special Event or Equipment Rental.

Expense/Reimbursement Request Form

The Expense Request Form must be filled out and approved before any expenditures will be dispensed from the Gethsemane UMC Finance Ministry. Church ministry leaders should complete this form and turn it in for approval before making any purchases for advance payment, or complete this form for reimbursement requests. If anyone other than the Ministry Leader is making a purchase, this form must be submitted with the Ministry Leader’s approval.

Van Request

This form is for the submission of requests to use of Gethsemane United Methodist Church Van for church ministry purposes and related events. A VEHICLE REQUEST FORM MUST BE FILLED OUT FOR EACH DRIVER. Request for use of the vehicle should be made as early as possible—no less than 1 week in advance.
